Successful Separation of Thoraco-omphalopagus Conjoined Twins, Preoperative Evaluation and Surgical Management


Abstract
This case report decribes our experience with a set of thoraco-omphalopagus twins, including a problem during preoperative period and surgical management.
Male thoraco-omphalopagus conjoined twins were separated successfully at the age of 59 days. They were transported to our hospital for surgical intervention 2 hours after birth.
Renal dysfunction of one baby(baby II) was appeared at the age of 5 days. The renal function of the baby II was compensated for by that of the other one.
There was no communication between the hearts at the atrial or Ventricular level and separate hearts by echocardiography. Liver union was confirmed by ultrasonography.
No communication between gastrointestinal tracts were found, and no other major anomalies were revealed.
The operation was rehearsed by team members including surgeons, anesthesiologists and nursing personnel before operation.
At the surgical separation, two completely independent individuals were revealed. Two separate hearts, two separate pericardiums, and two separate peritoneums, a union of the liver, common diaphragm were found. Two pericardiums and peritoneums
were
adhered tightly.
Cross-circulation between both babies might have occurred form the fused liver and thoracoabbominal wall.
Prosthetic sheeting(lyo-dura) was used to close the pericardial defect in baby II and the part of peritoneal defect of upper abdominal wall in both babies. Abdominal wall was closed with out difficulty.
